The cave diving community has a
long history of accident analysis that has been the primary force shaping the
training programs in place today. This accident analysis has been exclusively
focused on "incidents" with fatal outcomes. Very little analysis has been done
on incidents that do not end in a fatality.
Accident analysis is an exercise in forensics, requiring the
investigators to try to figure out how the victim got to where he was. Incident
analysis should prove to be a learning opportunity by allowing the participants
to share the "event cascade" leading up to the situation they are reporting,
helping others avoid the same problems. The CDF Incident Reporting and Analysis Project (IRAP) has
been started to establish a process to collect
this information and present it in a way to allow divers to learn from others
mistakes.
