Medical waiver?

Does the CDS require a Medical waiver before starting training? I've got a friend who is starting soon and it's not been mentioned and I thought I'd ask...... If yes, do they require on their own form, or do they accept other agency's?

I do not recall filling one out myself, but that was over 10 years ago.....

Yes, there is a medical waiver. I don't know any training agency that doesn't have one. It's not like PADI's form in which any yes requires a doctor's release, but if he was my student I would require a doctor's release with certain yes responses. Your friend should contact his instructor to get the form and ask these questions before heading out for training.

The RSTC form should do the job, no? You can download it from their website. Instructors, is this accurate?

The CDS is not governed by the RSTC. The instructor should be providing the correct forms to the student.
